Exclusions
All fees exclude laboratory / test costs and medications. Medications are charged separately at the pharmacy. Laboratory charges are based on the samples tested (blood, stool, urine, etc.) and billed at cost. Fees also exclude any third-party specialist or diagnostic charges.
